1. In the admissions packet you should have:
   
   1. Application for Nomination
   2. Information Sheet
   3. Academy Checklist
   4. Academy Address List
2. YOU ARE RESPONSIBLE FOR RETURNING:
   
   1. Completed Application (TYPED)
   2. Three Letters of Recommendation
   3. A Copy of Your ACT and SAT Scores
   4. An Official High School Transcript (An official copy with numerical class rank and school seal)
   5. A One Page Essay, "Why I Would Like To Attend A Service Academy" (See page 3 of application). Essays more than one page or not submitted on the original application form will not be accepted.
3. YOU ARE RESPONSIBLE FOR AND MUST:
   
   1. Return your completed application packet prior to the deadline of NOVEMBER 3RD. APPLICATION NOT POSTMARKED BY NOVEMBER 3RD MAY NOT BE CONSIDERED.
   2. Immediately request and remit a Pre candidate Questionnaire (PCQ) card to each Service Academy you are interested in attending (PCQ Evaluations will be reviewed by the screening committee).
   3. Supply the respective academies with any documentation required for the admissions process. NO DOCUMENTS WILL BE FORWARDED BY THIS OFFICE.
